Please fill out the 5Essentials survey as soon as possible. You will need to fill out a survey for each school in the District in which you have a student enrolled. See this letter  for more information about the survey.

You should choose the Parent Survey in English, as marked below.

Then you can enter your email, County in which you live (Kane), and search for the school, as shown below.
